Telehealth Education and Training for the Clinical Workforce

- Vendor and platform training. Necessary for operating a specific system, but not a substitute for clinical telehealth education — knowing where the mute button is does not teach a virtual abdominal exam.
- Professional society guidance. Specialty societies and the American Telemedicine Association publish practice guidelines that function as self-study curricula for specialty-specific virtual care.
No certificate confers legal authority to practice telehealth — licensure law does that — so the value of any paid program lies in skills gained and documentation produced, not credentialing magic. Evaluate programs by category and content rather than brand.
Competency frameworks
Training needs a target, and the field has converged on published competency frameworks to supply one. The most influential is the Association of American Medical Colleges' telehealth competency framework, which defines expectations across six domains — patient safety and appropriate use of telehealth, access and equity, communication, data collection and assessment via telehealth, technology, and ethical and legal considerations — at three developmental stages, from entering resident to experienced faculty physician. Designing a health-system training program

- Baseline for everyone. A short required module covering platform operation, documentation and consent requirements, privacy practices for home-based work, and emergency protocols — completed before the first solo virtual visit.

Well-designed telesimulation cases test what in-person OSCEs cannot: guiding a patient through self-palpation, managing a scripted connection failure, handling a safety disclosure when the patient is alone at home, or conducting a visit with a caregiver interpreting. - Knowledge assessment on legal, privacy, and appropriate-use content — the material that changes most often and fails silently.
